Personal Injury Protection: Each Utah insured driver automatically has a minimum of $3000 that can be used for chiropractic and other services regardless of who was at fault. This benefit is extended to each occupant of the vehicle at the time of the accident. You may have additional coverage beyond the $3000 if you have that added to your policy. Call us first.

Under Insured Medical Coverage

If you carry UIM coverage, your attorney may be able to access those funds on your behalf.

Bodily Injury Coverage

BI coverage is the insurance of the at-fault-party which covers injuries to you and your passengers. This coverage is accessible to cover additional medical care if your PIP benefits are not sufficient to fully diagnose and care for your injuries. In addition to covering medical costs, BI may also cover lost wages, pain and suffering and can be accessed for you by a competent attorney.

Worker's Compensation

At Spine Craft we can bill your employers insurance at NO COST TO YOU if you are injured at work.

State Worker's Compensation

State employees have mandatory worker's compensation insurance if they are injured on the job.

Private employer Work Comp

In Utah it is mandatory that every employer carry Work Comp Insurance on each employee. All we need is a claim number, billing address and phone number to file your medical care if you were injured on the job.

Federal Worker's Compensation

Spine Craft is one of the few clinics that is registered and able to bill under Federal Worker's Compensation in Utah. We will need a claim number to file the claims on your behalf.

Private Health Insurance

We are often able to provide care for your normal copay. Call to see if you qualify.

Our office is enrolled in Select Health Network. Send us your policy number, your name and date of birth and we can check your in network benefits. Some plans have chiropractic benefits with a set copay, others have no chiropractic coverage. Some plans have a percentage based chiropractic coverage. Those that do have chiropractic benefits have 25 treatments a year without preapproval. We will need your ID number, name, and date of birth to be able to check on your coverages.

HSA Cards Accepted

You can use your HSA card just like a debit or credit card at our office to cover the care you need.

Other Private Insurance

We continue to research and evaluate accepting new insurances. However, our self-pay rates are often near or below many insurance company copays. Some of our services are not covered by some insurers such as shockwave therapy, massage therapy and laser therapy. Benefits of paying at the time of service: 1. No deductible, 2. No surprise bills for uncovered services. 3. No limitations on care. 4. Significant discounts. 5. We can provide you a billable receipt to send to your insurer

Care Credit Accepted

We are enrolled in and accept Care Credit. If you already have an account or if you apply and are accepted you can get the care you need and make monthly payments with zero interest. Terms apply.
